Agent of Record Letters

Please contact CAAIS before beginning the AOR process with your client. The following information will be needed to submit for consideration:

• • Carrier Name(s)
• • Name Insured(s)
• • Policy Number(s)
• • Effective Date

Once CAAIS confirms acceptance of the AOR, please have your client complete and sign/date the downloaded form on their letterhead.
